Introduction- Straatsma introduced a syndrome of ipsilateral extensive MRNF, anisometric myopia, amblyopia and strabismus in North Americans & Kodama found that in Asian subjects from The Gutenberg Health Study (GHS). Case report: A 35 yr/F came with C/O DOV in LE since 6 months with H/O trivial trauma. O/E Vn in LE CF ½ meters PH 6/60. Her BCVA was 6/24 with -8 Dshp. A/S normal & pupil was sluggishly reactive to light , no RAPD. On examination extensive Myelinated nerve fibres around peripapillary area, Vitreous opacity/tag was present in front of the disc. No FR & Myopic background retina. RE Vn 6/6 A/S & fundus WNL . B scan of LE showed Vitreous opacity measuring 1.6×1 mm in front of disc and axial length of 23.1mm. Occlusion therapy was started, follow-up for 6 months with no Vn improvement. Conclusion: This rare syndrome is practically resistant to therapy. Early Dx of this constellation in first few yrs of life & early therapy can lead to good Vn. Financial disclosure: None
