Introduction: The lower lid tumours & its traumatic partial loss are reconstructed by Hughes flap procedure. It’s cosmetic & functional results in Indian settings are not known. Purpose: To evaluate cosmetic results, functional outcome & postoperative complications after the Hughes procedure in an Indian scenario. Methods: This was a retrospective observational study of the operated cases of Hughes procedure. The patients were evaluated for cosmetic results, postoperative symptoms, ocular surface examination, tear film stability, meibography, tear production as well as postoperative complications. Results: All 20 patients in the study had 100% cosmetic results. Tear film instability was noted in 17, ocular surface abnormalities in 12, significant meibomian gland loss in 12 & postoperative complications in 3 cases. Conclusion: In spite of favourable cosmetic results, Hughes procedure does have limitations in the functional outcome emerging out of tear film instability.
