Introduction- VOGM, a rare congenital malformation of VOG from failure of regression of median procencephalic vein & venous obstruction at transverse /sigmoid sinus.
Usual presentation is cardiac failure at birth. If survives infancy, presents with hydrocephalus, seizures, delayed milestones, Parinaud’s syndrome etc & in adults SAH from rupture of VOG.
Materials and methods- Presenting a 11 month child with developmental delay &seizures for a month. Pupils were equal, reacting & LE disc edema. In CT brain dilated supratentorial ventricles& mural VOGM with fistula from posterior choroidal artery in DSA .
Supraselective catheterisation of left posterior choroidal artery & fistula embolisation done by Interventional Radiologist. Child did well post procedure. Papilledema and systemic problems were expected to regress in a month.
Conclusion : Papilledema is an unusual manifestation in 11 month old. Timely endovascular intervention results in resolution of ICP and normal development.
