To report ‘Drusen ooze’ in 4 eyes with dry AMD presenting with subretinal fluid. Large confluent drusens were closely followed up with multimodal imaging (OCT ,FFA,ICG ) over a period of 4 – 8 years. Initial vision of patients with large drusenoid PED was 6/9 – 6/7.5. Sub retinal fluid developed with fibrinoid and pigment deposits under the retina. FFA and ICG did not reveal any definite choroidal neovascular membrane. Trial of anti VEGF injections in 2 eyes did not dry the fovea. Drusen ooze was diagnosed and patients were observed thereafter with maintained vision and trace subretinal fluid. OCT Angiography did not reveal any vascular network. It is a unique entity which presents with migration of drusenoid material and pigments in the subretinal space through the breach in RPE and may not be VEGF driven and results in reduction of PED size. It may remain stable over years and multimodal imaging especially OCTA can help in distinguishing dry from wet component of AMD.
