Multiple idiopathic serous retinal pigment epithelial detachments (MISRPED) represent an uncommon entity with manifestations ranging from asymptomatic to breakthrough vitreous hemorrhage. We describe a 44 years old male patient who presented to us with sudden diminution of vision in the left eye. Fundus examination revealed MISRPED in the right eye and vitreous hemorrhage with massive subretinal bleed in the left eye. Indocyanine green angiography of the right eye revealed polyps with hypofluorescent halo pointing towards inception of polypoidal choroidal vasculopathy which is a well-known cause of subretinal and breakthrough vitreous hemorrhage. This suggests a temporal sequence of events leading to subretinal hemorrhage in MISRPED. In another case, we describe the early manifestations of the latter disease process. Thus our original work unravelled the mystery of vitreous hemorrhage which has never been described in literature.
