Purpose: CSR target, is often based on what is feasible rather than what is needed. This study is designed to estimate the annual need for cataract surgeries in a given population, reflecting this reality of what is ‘demanded’ and what is ‘needed’.

Methods: Prospective population based cohort design done in Theni district. 10144 subjects, 40 years and above were enrolled from 34 clusters using “Cluster Randomized Sample”.

Results: 7127 subjects (70.3%) completed the eye examination. The mean (SD) age of the subjects was 54.8(10.9) years and 42% were Males. Of the 7127 subjects, 240 subjects were advised for cataract surgery and 147(61%) had undergone cataract surgery in 12 months. Eye assessment showed 645 (9.1%) subjects as visually impaired and 20 (0.3 %) as blind due to cataract. Projecting this result to the Theni district population shows a CSR of 8800 and CSC (eyes) of 87% for VA<6/60. Conclusions: This novel study design helps in projecting the annual cataract surgery need.
