FP1403 : Management of a case of recurrent conjunctival viral papilloma

Synopsis-

A young male presented with a swelling in the right eye caruncle since 2 months.On examination a fleshy mobile mass attached to the underlying surface was seen.Excision biopsy,cryotherapy and amniotic membrane grafting was done.Biopsy report suggested viral papilloma.Interferon alpha 2b eyedrops in the concentration of 1000 millionIU/ml was started.On follow up amniotic membrane shrinkage occurred.Covering the defect with conjunctival autograft and continuation of interferon drops failed as there was recurrence of caruncular swelling.He was started on adjuvant mitomyin c(MMC) eye drops following which there was remission

Conclusion-Conjunctival viral papillomas are benign but are highly recurrent (27%).To treat recurrences various methods used are CO2 laser, oral cimetidine,MMC and interferon alpha 2b.Early detection of failure of monotherapy and initiation of combination drugs like MMC with interferon alpha is the key to treat multiple recurrences.
