We report a rare case of ruptured lipoma presenting as a fourth cranial nerve palsy. A 27-year female presented with headache, vertigo, and vomitting. On examination both eyes best corrected visual acuity was 6/6. Both eyes anterior segment normal . Rotatory Nystagmus was present in both eyes. Right Fourth cranial nerve palsy was noted. An MRI of the brain revealed Intracranial fat intensity lesion involving posterior inter hemispheric fissure with a subcutaneous scalp extension through the bone defect. Multiple fat intensity focis scattered in the brain – Ruptured lipoma / dermoid and persistent falcine sinus.we ae presenting this case for its rarity.
