Purpose: To evaluate the expression of Toll Like Receptor 2 (TLR2) and Toll Like Receptor 4 (TLR4) on CD14+ cells in post operative endophthalmitis(POE) patients. Methods: Prospective case control study of 16 patients of POE who underwent 23 G Parsplana Vitrectomy (PPV). Ten patients undergoing 23 G PPV for non infectious conditions were taken as controls. Undiluted vitreous samples were collected in all cases . Simultaneous venous blood sample was taken and flowcytometry was performed to detect the expression of TLR2 and TLR4 in Vitreous and blood samples. The patients were kept under close followup and visual acuity was assessed at 4 weeks after surgery. Results: TLR2 and TLR4 expression in vitreous and not blood was higher in POE than controls. A drift towards higher level of expression of TLR2 and TLR4 in vitreous was observed in patients with poor visual outcome. Conclusion: The management of POE may be targeted at the downregulation of TLR2 and TLR4.
