A 30 year old female presented with sudden onset diminution of vision in right eye. Her BCVA (Snellen) in right eye was 6/18, and left eye was 6/6. Her slit lamp examination and IOP were within normal limits. Right eye fundus examination showed presence of an optic nerve head granuloma with peripaillary subretinal fluid. Ultrasound Bscan of right eye showed a localised chorioretinal elevation in optic nerve head area which was also seen on spectral domain OCT along with SRF. Fundus Fluorescein angiography showed optic nerve head hyperfluorescence due to dye leakage. Her Mantoux test was 20×20 mm and interferon gamma release assay was positive. Serum ACE levels and chest x ray were within normal limits.The patient was started on first line anti tubercular therapy followed by oral steroids at 60mg/day in tapering doses. The patient responded very well and her BCVA improved to 6/6 in right eye at 4 months follow up. Fundus examination also showed complete resolution of right eye lesion.
