Male patient diagnosed with LE cataract & underwent Femtosecond Laser-Assisted Cataract Surgery.During irrigation & aspiration procedure, capsulotomy margin tore & extended to equator, but surgery well managed & a foldable IOL placed in the bag.

During viscoelastic removal, patient felt uncomfortable and without warning the patient vomited. The vomitus entered into the operating field through inner canthus despite a proper drape. Paroxismal vomiting & retching went on for 10 minutes.Thorough wash of conjunctival cul de sac done with BSS.The patient was discharged when systemic condition stabilized. Moxifloxacin, tobramycin and steroids eye drops were prescribed post operatively. The patient reviewed twice daily for 7 days & on alternate days for 2 weeks and then monthly for 3 months. The conjunctival culture reports negative.

On slit lamp examination no AC reaction seen, BCVA 6/6 & N6 and patient was doing well despite gross contamination of cul de sac during final stage of surgery.
