Tuberculosis is the first infectious disease to be declared by W.H.O. as a global health emergency and it is a widespread epidemic in our country. Due to lack of awareness, atypical presentations and delayed treatment, it has a very high rate of morbidity. We present a series of 28 cases who presented to general ophthalmology OPD in 2017 (Jan – Dec) in a tertiary care center in south India. Common complaints were defective vision and headache and on thorough evaluation unfolded a plethora of diagnoses including optic neuritis, sub retinal abscess, choroiditis, papilloedema, nerve palsies, optic disc granuloma etc. While few were known koch’s patients, some were diagnosed with intracranial TB based on our investigations. This case series with documented clinical signs, rare photographs and atypical presentations will improve awareness and give a clue as to when and where to suspect TB.
