Dr.Santanu Das, D20971

SYNOPSIS-A 24 year old male patient presented to with history of gradual protrusion of the right eye ball since 6 months.

On examination axial proptosis was noted. There was no restriction of ocular motility and diplopia.Vision was normal with no rapd.MRI orbits with contrast depicted a well defined intraconal mass abutting the lateral rectus muscle and enhancing post contrast.Lateral orbitotomy with excision biopsy was done.Histopathological report suggested benign fibrous histiocytoma.Post operatively there was no recurrence with resolution of proptosis.

CONCLUSION-Fibrous histiocytoma can be benign, locally aggressive or malignant. If not completely excised the benign forms may become malignant.Usually the least aggressive or benign form will present with slow onset of proptosis. Malignant form can present with rapid onset of proptosis accompanied by diplopia, pain, chemosis, motility restriction. Complete surgical excision is the treatment of choice.
