A 22 year old young male,came with diminution of vision in both eyes.On examination his BCVA was in RE 6/9,N6 and LE-6/6,N6.Anterior segment showed alternating exotropia,krukenberg spindle was found on corneal endothelium ,deep anterior chamber.His presenting IOP-RE 36 and LE 24mmHg.Fundus examination-CDR RE 0.9 with bipolar rim thinning and LE 0.6.Gonioscope revealed open angles with heavily pigmented trabecular meshwork.Humphrey Visual fields showed right eye-tubular vision,left eye-paracentral scotoma.OCT RNFL-right eye-superior,nasal and inferior quadrant thinning.Left eye-temporal quadrant thinning.A diagnosis of both eye pigmentary glaucoma was made.Patient started on brimonidine(0.2%) +timolol(0.5%) BD and latanoprost(0.005%)HS both eyes.On subsequent follow up his IOP reduced to 8mmHg in both eyes.This case shows importance to rule out pigmentary glaucoma in young male patient.Also strict adherence to medications and, follow, needed for control of glaucoma progression.
