Purpose: To investigate endothelial imaging patterns in ICE syndrome and corelate with the clinical features. Methods: 25 subjects with a diagnosis of ICE syndrome were studied. Results: Mean age was 47 years; 13 were males, 12 were females. All patients had clear central cornea and reasonably good vision. Endothelial abnormalities were noted in all. Endothelial abnormalities were documented using Sherrard’s classification system. The most common type was Subtotal ICE. Conclusions: Specular microscopy provides valuable insight into the endothelial abnormalities. These have implications in the management of ICE syndrome
