Diabetes mellitus and retinitis pigmentosa(RP) occur independently but few cases of diabetic retinopathy(DR) in retinitis pigmentosa are reported which were associated with Bardet Biedl,Alstorm,KearnSayre or Wolfram syndromes. A 60 year diabetic male with unilateral retinitis pigmentosa for 35years presented to us with progressive diminution of central vision in both eyes. His visual acuity was finger counting in right eye and 6/60 in left eye. Fundus of left eye showed retinal hemorhages and exudates and clinically significant macular edema on OCT.Patient did not take any treatment for two months and came with further diminution of vision in both eyes. Right eye showed hemorrhages and exudates in central normal retina and increased exudation in left eye. Anti VEGF was given in his left eye and visual acuity improved to 6/18 at one month followup. DR developing in a case of RP not associated with any syndrome is rare and demands for a further look deep into the pathological process.
