Five patients with Eso DRS were evaluated for PP deviation, globe retraction, abduction limitation, AHP, shoots, V pattern and fundus torsion. Transposition of SR to LR, recession of LR 6 to 8 mm, large recession of contralateral MR was undertaken. Age ranged from 4 to 16 years, there were 2 males and 3 females. PP alignment was within 8 PD, horizontal eso-correction from 15 to 30 PD was obtained, abduction was improved by 1 to 2 grades. Globe retraction, AHP, shoots were also ameliorated. V pattern worsened in 2 cases. Subjective fundus extorsion of 4, 6 and 7 degrees was induced in 3 patients. No new vertical deviation was induced, neither any adduction limitation in contralateral eye. SRT to LR with simultaneous recession of LR and large contralateral recession of MR seem to additionally improve globe retraction with amelioration of abduction, achieve eso correction of 15 to 30 PD and AHP. Worsening of V pattern and subjective torsion may happen. No such reports exist.
