Aim:To evaluate necessity of neuroimaging in third nerve palsy(TNP).Methods:Prospective observational analysis of TNP(Isolated and Non-isolated) cases.Results:Analysis of 60 cases showed male preponderance (68%) with mean age 50.5 ±16.07 yrs. Of 60 cases, isolated(48)& non-isolated(12); complete(9)& incomplete(51);pupil sparing (26)& pupil involving (34) cases. Most common etiology ischemic in 43%, inflammatory 18%, traumatic 15%, idiopathic 10%.48 cases underwent MRI brain, in 29 cases scan was diagnostic of the etiology, giving diagnostic yield of 60.4% for TNP patients, maximum for compressive etiology followed by inflammatory & traumatic. We found some rare causes like Pagets disease leading to skull thickening, compression due to closely related normal posterior cerebral artery, associated bilateral retrobulbar neuritis. We did not had any posterior communicating artery aneurysm.Conclusion:Neuroimaging plays important role in evaluation, diagnosis & treatment in third nerve palsy
