A 10yr old girl presented with diminution of vision in Both Eyes(BE) for the past 3 years.Her best corrected visual acuity is 6/9,N6 BE.She had a family history of glaucoma in her paternal uncle.She wasn’t on any long term medications.Her intra ocular pressure(IOP) was 10 mmHg in BE.Slit lamp biomicroscopy revealed clear cornea with no haab’s striae.Corneal diameter was 15mm BE.Gonio revealed open angles with anterior insertion of iris.Cup Disc ratio was 0.7 in BE. Possibility of congential glaucoma with megalocornea was suspected and was evaluated further.Phasing revealed IOP difference of < 2mmHg.Axial lengths were 24.59 & 25.33 respectively.Pachymetry was 540,522 microns.No Visual fields defects.OCT RNFL was normal.In the absence of haab’s striae,normal visual fields and IOP a diagnosis of simple congenital megalocornea with suspicious disc was made.It should be differentiated from congenital glaucoma,as it is a surgical disease and requires prompt treatment to prevent blindness.
