Immune recovery uveitis(IRU) is one of the rare causes of vision loss in AIDS patients.We describe a case of IRU resulting in secondary glaucoma in a patient with HIV &Tuberculosis(TB)
CASE:A 33year old HIV positive male on irregular treatment presented with pain,redness and defective vision in left eye(LE) for one week.Examination revealed HIV retinopathy in both eyes with yellowish choroidal granuloma & subretinal fluid with vision of 4/60 in LE.He was referred to ART centre for HIV &TB evaluation and was started on Antiretroviral therapy(ART) & Antitubercular therapy(ATT).Two months later he came with LE pain for four days. Examination revealed vitritis in LE.Oral steroids was started.Eventually he developed LE neovascular glaucoma with no light perception
CONCLUSION:We report this case to emphasize that initiation of ATT & ART together can worsen complications.WHO 2017 guidelines recommend “TB treatment should be initiated first followed by ART within first 8 weeks of treatment”
