A 4 day old premature (33weeks) neonate in sepsis (Klebsiella) was referred with left sided leukocoria & cloudy cornea. There was lid edema, circumcorneal congestion, buphthalmos & hyphema. Other eye WNL, Bscan suggestive of intraocular mass with calcific specks. MRI showed laterally ruptured globe,retinal mass involving lacrimal gland & lateral orbit, chorioretinal hemorrhages & RD with calcium specks. No pineal tumour. Bonemarrow, CSF was negative &steroids were given to control inflammation. Subsequently extended enucleation with implant was done. Histopathology revealed inflammatory pseudotumor (No Rb). The child underwent prosthetic rehabilitation. Leukocoria with calcific intraocular mass directs suspicion towards RB. In case of a disfigured blind eye with nil visual prognosis, enucleation& histological diagnosis aids treatment planning. Preoperative steroids can be used to settle the inflammation & improvise surgical outcomes.
