Papilledema is defined as a non inflammatory swelling of the optic disc secondary to raised intracranial pressure. Here we report a case of a 36 yr old female who presented to our neuroophthalmology department with primary complaints of severe bilateral headache and progressive blurring of vision in both eyes since past 2 weeks. A thorough clinical examination was done including visual acuity, visual field charting, assessment of extra ocular movements and fundus examination with both direct and indirect ophthalmoscopy. restricted upward and lateral gaze along with bilateral papilledema was observed. Investigations carried out included hemogram, MRI/MRV. primary hematological investigations suggested leukemia and acute myeloid leukemia was confirmed through PBS study. Studies suggested that ocular lesions were detected in asymptomatic leukemic patient, therefore we suggest ocular examinations to be included as a routine initial leukemic check up.
