Retrospective study done in 46 children diagnosed with cortical visual impairment. All underwent detailed ophthalmic and neurological evaluation with visual rehabilitation. Commonest causes were perinatal hypoxia (43.47%), neonatal hypoglycemia (8.6%), followed by prematurity and seizure disorder. Commonest MRI features were hypoxic ischemic encephalopathic changes (45.6%), periventricular leucomalacia (6.5%), bilateral occipital gliosis (8.6%), encephalomacia (6.5%), corpus callosum agenesis (4.3%). The commonest neurological deficits being seizures, global developmental delay ,microcephaly , cerebral palsy, West syndrome , etc .The ocular features being nystagmus , squint and disc pallor. Vision improvement was seen in 1/3rd of the patient inspite of their etiology but majority of them were still visually handicapped. Such patients require an integrated and an optimistic approach not just by pediatric ophthalmologist but also by their parents.
