PURPOSE
To report a rare case of optic nerve transection with total retinal detachment.
METHOD
A patient with complaint of unilateral complete loss of vision following road traffic accident presented after 3 months of the incident. He had fractures of floor, medial and lateral wall of left orbit along with fractures of all the walls of maxillary sinus on left side. He had a vision of 6/6 R/E and no perception of light in L/E. Direct pupillary light reflex was absent in L/E. Indirect ophthalmoscopy and fundus photography was done which was suggestive of total retinal detachment. CT scan was suggestive of retro bulbar optic nerve transection.
RESULT
Dilated fundus examination showed optic nerve transection with retinal detachment involving the macula. CT scan was suggestive of discontinuity of retro-bulbar segment of left optic nerve which was confirmed on MRI.
CONCLUSION
Optic nerve transection is a rare presentation encountered in the patients of traumatic optic neuropathy.
