PURPOSE : To present a case of repeated TICL(Toric Implantable Collamer Lens) rotation post repositioning .
METHODS : A 24 year old female came with complaints of sudden diminution of vision in left eye after 2 months of uneventful TICL implantation in both eyes.TICL vault was less than half the corneal thickness (CT),250microns on Anterior segment OCT with 90 degrees rotation on dilated examination.She underwent Repositioning .4 months later patient had the similar situation.Undersizing was suspected and hence managed by a lens exchange.
RESULTS : At 15 month follow-up patient had vision of 6/6 with stable ICL position and good vault (1 CT on slit lamp examination,500microns on ASOCT).
CONCLUSION : Inspite of taking all important parameters into consideration which was similar in both eyes, undersizing was noted only in left eye.This indicates that the horizontal WTW diameter is not a gold standard for ICL sizing. Other modalities of assessment need to be explored.
