Phthiriasis palpebrarum is rare type of eyelid manifestation of Phthiriasis pubis. Phthiriasis palpebrarum is sexually transmitted or non-sexual when hygiene is compromised.
A 25 year old male presents with itching of right eye since 3 weeks, pain and insects crawling since 2 days.
Slit lamp examination revealed lice and nits anchored to eyelashes with multiple red pinpoint excretions, mild congestion and papillae in upper palpebral conjunctiva.
Some eyelashes, lice and nits were removed mechanically using flat tweezers.
He was referred to the Dermatologist and was detected to have lice in the pubic area. He was prescribed 1 % permethrim shampoo.
At the end of 2 weeks no louse or nits was present on his body.
In conclusion, eyelash and eyelids should be carefully examined as Phthiriasis palpebrarum is commonly misdiagnosed as conjunctivitis or blepharitis. Mechanical treatment is the most effective treatment with regular follow up to detect any residual lice and nits.
