– A 27 yr old male, plumber by occupation, presented with history of high voltage electric injury (11000 V) over his head. On examination patient was conscious and well oriented .He had third degree burns over scalp, and complained of marked diminution of vision . Significant ocular examination noted sluggish pupillary reactions with perception of light in both the eyes. Fundus examination showed marked bilateral disc edema with no associated retinal or disc haemorrhage, exudates or vessel abnormality. Neuroimaging and cardiac evaluation was within normal limits. Patient was given a trial of intravenous methyprednisolone for three days with tapered dose of oral steroids. There was rapid recovery of vision in both the eyes to 6/9. This case highlights the fact that electric current can induce isolated bilateral optic neuropathy, its early diagnosis and timely treatment with steroids.
