A 37 year old female presented to us with complaints of defective vision in her left eye since 2 months with associated left sided headache.She gave no other positive history except a recent delay in her menstrual cycles.Her BCVA in RE was 6/6 and in LE was ½ /60.IOP in both eyes was normal.Color vision and field testing were normal in RE and not possible in LE due to poor vision.Anterior segment showed RAPD in LE with all remaining findings normal in BE.Fundus examination in both eyes was normal and blood investigations also revealed no abnormality.With a diagnosis of retrobulbar neuritis we started her on iv methyl prednisolone 500 mg bd for 3 days followed by oral prednisolone 50mg OD.Her vision in LE improved to 6/9.Subsequent color vision testing was normal in BE and fields showed a temporal hemianopia in LE.Neurological evaluation was done and MRI brain with contrast revealed a pituitary macroadenoma with possibilty of apoplexy.We referred her to neurosurgery for tumor removal.
