Pediatric population rarely shows pituitary tumours. Amongst all pediatric brain tumours, these account for <3%. Here, we present a case in which a 10 year old child presented with defective vision. His vision was 6/60 which improved to 6/9 in the right eye and 5/60 which improved to 6/24 in the left eye. On examination, he was found to have partial ptosis in the right eye with -2 elevation defect with a mid-dilated sluggishly reacting pupil. A small amplitude nystagmus was noted. Fundus showed temporal pallor. A diagnosis of right-sided third nerve palsy was made and a MRI scan was requested. The MRI revealed a well-defined 3.3*2.6*2.3 cm mass in sella. There was a parasellar extension with encasement of the right cavernous sinus. On this basis, a pituitary macroadenoma was suspected. The patient was referred to the neurologist for further management and is doing well with medical management.
