FP408 : Effect of lateral rectus muscle Y split on globe retraction with up & downshoot in Duane’s syndrome

Introduction- The up & downshoots are seen in 25-39% of patients with Duane’s syndrome associated with severe co-contraction and globe retraction(GR). The up & downshoot occurs due to leash effect from the lateral rectus muscle. The study presents the effect of lateral rectus muscle recession (LRR) with Y split in the treatment of moderate or severe GR associated with up & downshoot. Methods-9 patients with Duane syndrome underwent surgery for +2 to +3 up or downshoot with abnormal head posture and 25-75% GR. LRR from 5.0 -12.0 mm with Y split was done in all patients with exo Duanes. In 3 patients with eso Duanes the medial rectus muscle was also recessed from 4.0 – 7.0 mm. Results-Mean age at the time of surgery was 13.5+ 9.5years.Median follow up was 6 months. After surgery, all patients showed nil to +1 up or downshoot, GR from nil to 50% and minimal face turn. Conclusion-LRR with Y-split is an extremely effective procedure in the treatment of up & downshoot with globe retraction .
