FP1045 : corneal melanoma rare presentation as corneal horn and management by an i-OCT) guided approach

A 43 years old female of Xeroderma pigmentosa, examined for eye redness, corneal mass lesion(6×7mm) for past 4 months, accelerated growth since 15 days. ASOCT and Ultrasound biomicroscopy was unable to delineate tumor-corneal interface because of shadowing.Under peribulbar anesthesia tumor was marked and dissected from surrounding 2 mm of conjunctiva and 1.5 mm of normal corneal epithelium. A blunt Sinskey’s hook and iris repositor was used to dissect the lesion at the level of tumor-corneal surface.Then under real time i-OCT guided images the corneal surface was dissected to achieve complete tumor free surface with out damaging Bowman’s layer.The healthy conjucntival margins were subjected to rapid freeze and slow thaw cryotherapy. Optimal results were obtained on day one with a best corrected visual acuity of 20/40.The cornea was clear with an intact Bowman’s layer on ASOCT .Unusual corneal tumor mass lesions under difficult situations can be optimally dissected off using i-OCT.
