FP669 : The diabetic screening card- a novel method to improve diabetic patient care and follow up.

Aim- Improve attendance and follow up of patient in the diabetic screening clinic

Materials and methods- Prospective study Sept 2016 to April 2018 .Patients screened for Retinopathy received a card with grade of retinopathy and date of follow up .The card was attached to the Medical book which was carried.The number of patients that turned up for repeat visits noted with reasons and grade of retinopathy noted.Results- 278 patients screened of which a 170(61%) returned by looking up the date card 22(7%) of these patient came for review as physician spotted the card and reminded patients about the repeat screening . 8%had moderate NPDR,1% severe NPDR and the rest no to mild NPDR

Conclusion- The introduction of a separate screening card serves as a good reminder for patients and in some situations the physician too can direct the patients . We recommend introduction of such a card at a national level which could ensure standardized quality of care and encourage patient follow up
