Introduction: Sympathetic ophthalmia is a rare, unilateral, granulomatous panuveitis that follows penetrating injury to one eye. It is a potentially blinding condition.
Method: In our case, a 40 year old male sustained a self sealing penetrating injury to his left eye. After 2 months the patient presented to our hospital with vision in right eye deteriorating to Hand movement & severe clinical manifestations of mutton fat keratic precipitates, peripheral anterior synechiae, aqueous cells and complicated cataract suggesting of sympathetic ophthalmia. Systemic antimetabolites & steroids along with periocular steroids were given & later SICS with IOL implantation for complicated cataract was performed in right eye regaining vision to 6/60 one month postoperative.
Conclusion: Although sympathetic ophthalmia is a rare disease, ophthalmologists should be aware of its devastating effects on vision & its prompt management.
