FP25 : Diffusion Tensor Tractography in predominant intrasellar versus parasellar Pituitary tumors

Role of diffusion Tensor Tractography (DTT) in pituitary tumors for prognostication and as an intraoperative guide is underestimated. We noted some peculiar differences in intrasellar versus parasellar pituitary tumors, although can’t be generalised at this point.

In pituitary tumors with dominant intrasellar component, chiasma is lifted up and pushed superiorly and posteriorly. There is reduction in postchiasmatic optic radiations. So early surgical intervention could be vision saving

Pituitary tumors with predominant parasellar component gets interspersed with chiasmatic fibres and postchiasmatic optic radiations are almost preserved in density. Surgical intervention in such cases means removal of chiasmatic fibres too and thus further deterioration of vision. Observation will give better visual outcome.

So MRI Tractography or DTT can help us in preoperative assessment and prognostication of case, which is entirely different in intrasellar versus parasellar pituitary tumors.
