A 16 years old Female presented with diminution of vision in both eyes since 6 months which was Insidious in onset, gradually progressive, associated with occasional headaches and pain in and around the eyes. The patient denied perception of light in the right eye. The Perception of light was present and projection of rays was inaccurate in superior and nasal quadrant of left eye. IOP measurement was 50mmHg (GAT)in both the eyes.Slit lamp examination of the anterior chamber revealed van Herick- grade IV in both the eyes.On Gonioscopy the angle was wide open in both the eyes.On Fundoscopy Cup-to- Disc ratio was near total cupping in right eye and 0.8 in left eye. optic disc was pale,narrowing of neuro retinal rim in inferior and nasal part with nasal shifting of blood vessels in both eyes(bayonetting sign positive). Trabeculectomy with Mitomycin C was done in the left eye. After 1 week – IOP measurements were 25mmHg in right eye and 12mmHg in left eye.
