Two females reported with c/o blurred vision & pain, signs suggestive of malignant glaucoma post phaco cataract surgery, operated elsewhere. B-scan-posterior segment WNL. First YAG PI & peripheral anterior hyaloidotomy through PI done with AGM & topical Atropine but in follow up, AC was shallow with one haptic of foldable IOL in sulcus tilting forward responsible for causing condition masquerading as malignant glaucoma appreciated only in dilated pupil. Redialing of IOLs in bag done in both cases. Post redialing VA improved, AC well formed & IOP become normal without AGM.
Case 1: RE VA-2/60,shallow AC, AT-54 mmHg with AGM, presented 2 mths post surgery.
After 6 wks of redialing BCVA-6/18, AC-NID, IOL in bag, AT-15 mmHg.
Case 2: RE VA-2/60, shallow AC, AT-30 mmHg reported 20 days post surgery.
After 4 wks of redialing BCVA-6/9, AC-NID, IOL in bag, AT-17 mmHg.
Conclusion: Tilting of foldable IOL can masquerade as malignant glaucoma, so carefully assess & do management accordingly.
