Vogt Koyanagi Harada is an autoimmune disease presenting with auditory, neurological, integumentary and ocular symptoms. VKH has a varied presentation. A 53 year old male patient presented with fever of 20 days, frontal headache and blurring of vision. He underwent investigations to uncover the root cause of fever which were inconclusive. A diagnosis of pyrexia of unknown origin was made by the phycisian and referred to our department for blurring of vision. On ocular exam, features of panuveitis, disc edema and multifocal serous retinal detachment were seen. Imaging modalities confirmed the diagnosis. The patient was started on intravenous steroids followed by gradual oral tapering of steroids. He responded well with resolution of clinical features and improvement of vision. As early and aggressive treatment prevent the formation of vision threatening complications, it is important to recognize the possibility of VKH in unusual clinical presentations.
