A 65 year old female presented with diminution of vision in right eye since 2 months following trauma with cricket ball. She presented with best corrected visual acuity (BCVA) of counting finger close to face in right eye and 6/6 left eye. On examination, her right eye showed decompensated cornea and subluxated IOL and normal intraocular pressure (IOP). Her posterior segment was not visible due to corneal haze. Her left eye was normal. She underwent right eye ultrathin Descmet’s stripping endothelial keratoplasty (DSEK) with intraocular lens (IOL) explant and modified Yamane’s Scleral fixated IOL under local anaesthesia. On 1 st post operative day she had BCVA of finger counting at 1 metre, lenticule well apposed and air bubble in anterior chamber with mild corneal edema and normal IOP. At 1 month, she had BCVA of 6/15 with +0.75 DSph -2.00 DCyl at 90 with near add of +2.50DSph N6. In this case we achieved good result by combing above the two techniques.
