Allergic fungal sinusitis presenting with visual loss is a rare entity. We report a patient with decreased vision in one eye later diagnosed to have allergic fungal sinusitis leading to compressive optic neuropathy. Neuroimaging revealed bilateral large soft tissue mass lesion in fronto ethmoidal sphenoidal and part of maxillary sinus causing compression of the optic nerve. Patient underwent Functional endoscopic sinus surgery and Optic nerve sheath decompression. Histopathological examination of the excised mass lesion confirmed extensive eosinophilic infiltration and debris. Post operatively, marginal improvement in visual acuity was noted in view of long standing disease.
