Biopsy helped in confirmation of squamous cell caracinoma of lacrimal sac in a 6-year-old male child presenting as epiphora and medial canthal swelling. Tumor progession after initial response with neoadjuvant chemotherapy was seen. This case highlights the atypical age of presentation of this potentially fatal lacrimal sac malignancy. It further
emphasises the importance of knowledge of signs of lacrimal sac malignancy amongst ophthalmologists treating childhood epiphora where inflammatory etiologies are most common. A high index suspicion, thorough clinical examination and knowledge of signs of malignancy may avoid delay in diagnosis of this rare but life threatening malignancy. Neoadjuvant chemotherapy may have a potential role in downstaging the disease and allowing more conservative surgical management in these tumors.
