This is a rare case report of a middle aged male who came with complaints of bilateral firm rubbery masses underneath eyelids with proptosis since 12 years, with no other systemic involvement. Anterior segment was normal in both eyes with no restriction of extra ocular muscles. MRI brain and orbit suggested ?reactive lymphoid hyperplasia. Since it looked like orbital fat herniation and was visible at the conjunctival aspect, differential diagnosis of lymphoma and orbital fat herniation through the septum were suspected. Incisional biopsy was thus done which showed xanthogranuloma orbit which was confirmed by histopathology and immunohistochemistry. Although it is a benign condition, we had planned for cosmetic improvement for which the patient was not willing for. As xanthogranuloma usually involves the medical canthus in the upper lids, the rarity of the location of this poorly understood tumour has perked my interest in this case.
