40 years male presented with painless gradually progressive diminition of vision both eyes with episodes of transient visual obscuration lasted for 1-2 min occuring multiple times from last 2 months, and also dull aching pain over face. Patient also had history of unilateral deafness for 4 years. On ocular examination visual acuity in both eyes are severely diminished, in right eye it is finger count close to face and in left eye hand movement close to face. Both eyes colour vision defective. Extraocular movements are full and free in all gazes. Relative afferent pupillary defect present in left eye. On fundus examination papilloedema of 4 diopters present in both eyes with disc pallor and engorged vessels. On systemic examination patient has sensory neural hearing loss in left ear, and LMN type facial palsy on left side. On investigating his MRI brain with contrast reports shows features suggestive of left cerebellopontine angle schwannoma with associated obstructive hydrocephalus.
