The differential diagnosis for a swollen disc can be extensive.With causes ranging from the infectious to the vascular to the malignant, the swollen optic nerve is a crucial and the patient’s vision or life may depend on a timely diagnosis.38 y old female presents with sudden onset painless loss of vision in BE since 10 days associated with rt.sided headache and h/o transient obscuration of vision (RE) 4-5 episode lasts for~30 min.On examination his BCVA : RE (PL+), LE ( FCF) . Anterior segment (BE) – normal , Fundus (BE) shows B/L disc edema with engorged & tortuos peripapillary blood vessels Macular star (RE) & mild vitritis (LE) B/L macular edema superficial & few dot & blot hemorrhage with hard exudates (BE) differential diagnosis of papilledema,neuroretinitis,infitrative optic neropathy,diabetic retinopathy or hypertensive retinopathy were considered. After thorough physical and ocular examinations and investigations diagnosis of grade 4 hypertensive retinopathy was made.
