A 27 year old high myopic female patient visited our clinic with complaints of worsening of vision post refractive lens exchange surgery. She had undergone the surgery 6 months back and noticed that her distance and near vision had gradually become foggy and was not getting respite with glasses and eyedrops prescribed to her. Her vision was recorded as 6/6, N6 in both eyes. Colour vision was within normal limits. Pupils were sluggishly reacting but bilaterally equal. Confrontation revealed a bitemporal hemianopia and a perimetry test was advised. Probing on the history further, she complained of chronic headaches and irregular menses. An MRI brain with contrast with special attention to sella, pituitory and optic chiama was advised which revealed a large fluid filled cystic lesion, probably a Rathke’s cyst compressing the chiasma. Trans-sphenoidal surgery was perdormed by the neurosurgeon. There was commendable improvement in fields and vision 3 weeks after surgery.
