PURPOSE:The aim of this study is to prove the ambiguity of Hutchinson’s rule -“if the side of the tip of the nose is involved the eye is also involved”.METHODS:This was a prospective study conducted in our department in the last 2 years.It included 68 patients of acute herpes zoster opthalmicus.They all were examined extensively with regard to ocular inflammatory signs and changes with 3 months of follow up.RESULTS:A total of 68 patients of herpes zoster were examined.It was found that out of 68,6 of them presented with nasociliary nerve involvement.Out of 6,3 of them (50) % though had nasociliary nerve vesicles on the side of nose but the eye was quiet showing no corneal or any significant ocular involvement.CONCLUSION:The present series though small,tend to disprove the classical concept of hutchinson’s rule.The rational conclusion can be that if the side of the nose is involved and more so the tip one should be in the lookout for ocular involvement more carefully.
