Radiation retinopathy & neuropathy are often overlooked since their clinical features mimic common ocular conditions in routine practice. Our 1st case was 50yr old diabetic lady presented with sudden worsening of NPDR (had bilateral multiple cotton wool spots and macular edema). Careful history revealed that she had received EBRT (30gy) for brain metastasis 1 year ago. Another 55yr old diabetic presented as asymmetrical DR with right eye neovascular glaucoma and left eye mild NPDR. He had history of radiation therapy (RT) for right side chondrosarcoma of paranasal sinuses 3years ago. 3rd was 50 year old man presented with bilateral sequential sudden vision loss which was poorly controlled with IV steroid. History revealed EBRT therapy for nasopharyngeal carcinoma. Nerve enhancement was noted on MRI suggestive of retrobulbar neuritis. High degree suspicion is needed to recognise such entities especially in the background of diabetes & DR where such entities are easily overlooked
