PURPOSE:To compare optical grade therapeutic keratoplasty(OpTPK)done within15days(gr1)and after15days(gr2)of onset of symptoms.METHODS:A study done in 32eyes with keratitis,underwent OpTPK and completed 1yr,divided into Gr1(n=13)andGr2(n=19).Tissues with cell count of more than 2000/mm2 were used,topical steroids were started from POD3.Clinical outcomes were recorded over 1yr.RESULTS:VA at each time(preop,PO1/3/6months&1yr)was compared between2groups.There was no difference in VA except for preop VA(gr1 poorer).Intraop complications were less in gr1-7.6% than gr2-15.8%.Rejections similar in gr1(15.4%)and gr2(15.8%).Re-infections were less in gr1-7.7% than gr2-26.3%.Failures in gr1-38.4% were higher than gr2–31.6%.After1yr f/u,46.1%grafts in gr1and58%grafts in gr2were clear.CONCLUSION:Early intervention has less complications and recurrence of infection.VA and rejections were independent of the duration of intervention.OpTPK provides therapeutic/optical results,avoiding second surgery.
