Method: A 73year old man who underwent phacoemulsification surgery in his left eye 3 weeks before was diagnosed with acute postoperative endophthalmitis and tunnel infitrate. He underwent Pars plana vitrectomy and intravitreal antibiotics.Vitreous biopsy, anterior chamber tap and sample from tunnel infiltrate did not yield any organism. As the infiltrate was persisting even after 3 months a tunnel excision with corneo patch graft was done.Graft was utilized from the remaining anterior cap of the tissue used for DSAEK(Descemet’s Stripping Endothelial Keratoplasty).
Results:After 5 months UCVA was 6/6 with complete resolution of his symptoms .
Conclusion:Surgical management of sclero-corneal tunnel infection with partial thickness corneal patch graft is a definitive treatment of localised non resolving wound infections wherein globe integrity can be preserved and good visual outcome achieved.
