36 year female diagnosed a case of congential cataract with microcornea underwent lens aspiration in both eyes & left aphakic at age of 5 years. Subsequently patient underwent aciol implantantion bilaterally. Postop period was uneventful. Patient developed watering, pain, redness, poor vision in left eye & reported to our centre. Examination revealed right eye as post op DSEK status and left eye had multiple subepithelial defect, persistent stromal edema suggestive of pseudophakic bullous keratopathy, expected difficulties and challenges encountered in this case due to microcornea & aciol, DSEK was considered in left eye & tapering dosages of topical steroids, antibiotics, cycloplegics & lubricants. At the end of 6 months, endothelial graft was taken up by the host cornea, specular microscope endothelial count of 2400 cells/mm, AS OCT revealed DSEK lenticule thickness of 178um and the patient attained a best corrected visual acuity of 6/18 in the left eye.
