A Case report of 60 Year old male patient presented to our hospital in the year 2012 with diminution of vision in both eyes for 6 months duration. He was diagnosed with both eyes Fuchs Endothelial Dystrophy /corneal edema with Pseudophakia , RE vision was 1/60.LE vision was 6/24 p, RE graft failure . Left Eye was only Hand Movements with Severe stromal scarring ,hence LE PKP was done on 24/05/2016. Surprisingly he had postoperatively Retained Old Descemets Membrane . Literature Search reveals so many reasons and treatment options for Retained old DM .
In our case we considered for conservative management with YAG laser initially which had aggravated graft failure but surgical intervention of DSEK with Retained DM removal is the appropriate treatment .
“Take Home message” from our case is that “ Though Retained DM is a Rare Complication Of PKP , it can occur , must be intervened early for surgical Descemetorhexis to safe the graft” .
